The London Devilettes Girls Hockey Association policy in relation to "Police Checks" for coaches, bench staff, dressing room volunteers, and on ice helpers is as follows to ensure we are in alignment with OWHA Screening Policies.

Year 1    Vulnerable Sector Check (VSC)

Year 2    Criminal Offence Declaration Form (CODF)

Year 3    Criminal Offence Declaration Form (CODF)

Year 4    Vulnerable Sector Check (VSC)

Year 5    Criminal Offence Declaration Form (CODF)

Year 6    Criminal Offence Declaration Form (CODF)

The policy runs in a three year cycle.  The first year a volunteer meeting the criteria (working with children 8 to 17 years) will be required to obtain a Vulnerable Sector Check (VSC).

For next two years of the cycle the volunteer must print out, complete and sign the
Criminal Offence Declaration Form.

The London Devilettes will accept police checks used for other organizations (eg soccer, baseball) as long as the police check was obtained within the 6 months prior to submission to the London Devilettes.

Anyone that has been away from the organization for a period of one year or more must re-submit for the “VSC”.

Anyone that has a police check on file with the London Devilettes dating back to 2019-2020 season will be required to submit for an updated “VSC”.

The head coach will be responsible for collecting the police checks/declarations from all the members of their team staff and then submit to Cassandra Cameron at [email protected] no later than September 30th. Failure to submit the required forms will result in an immediate suspension of the individual until the required form is received.
