Competitive Coaching Application - 2024 / 2025 Season
Prior to completing the form please read the instructions carefully. 1. Please ensure all required fields are completed before submitting. 2. In order for the application to be considered, the section "Coaching Philosophy" must be completed. 3. The intent of the coaches' selection program is to identify the best available coaches to lead our most valued asset on and off the ice. As a result, a coach may be removed from their position at any time if it is determined that they are not meeting the expectations of the Board of Directors. 4. Successful applicants will be required, from time to time, to attend association coaches meetings and/or clinics. 5. While the London Devilettes Girls Hockey Association requires applications be submitted online to ensure deadlines are met, completed applications can also be scanned and emailed directly to the London Devilettes President at [email protected] Application deadline for all competitive teams is Friday January 10, 2025 @ 11:59 PM.

Team Selection

Please select the team you wish to coach. Please note: The organization intends to offer multiple teams at the same level if there is sufficient numbers and skill level to support it

If you have a daughter eligible to play on the team that you are applying for, an independent evaluation of your daughter's skills, to ensure that she is the skill level to play on the the team may be completed.  By continuing with this application, it is understood that you agree fully to this point and consent to an evaluation. 

Coaching Certification

Applicants interested in being an assistant coach with one of our High Performance Teams at U18AA, U15AA or U13AA for the upcoming season must already have a High Performance Stream (HP1) certification prior to applying for this role or obtain it before the season begins in August 2024. These applicants are asked to review the course requirements to ensure they are comfortable committing to taking the course.  The LDGHA will consider financial assistance for the HP1 course for those applicants successful in securing a role with one of our High Performance Teams.

Questions or concerns on the certification requirements or the HP1 course can be directed to the President and/or Competitive Director.

Please provide previous coaching experience beginning with the most recent season.

I give permission to these persons to provide information about myself to the London Devilettes Girls Hockey Association.

PLEASE NOTE: The selection committee will review all applications, but not all applicants will be asked for an interview.

The LDGHA reserves the right to replace or suspend a coach at any time.  The successful coaching candidate will be required to sign an acknowledgment form on the terms of their appointment and operate under budgetary and time guidelines. The successful candidate will also acknowledge that the LDGHA Board of Directors will appoint an independent evaluator to attend each tryout who will represent LDGHA and will assist in team creation and player selections for each age group.
The London Devilettes Girls Hockey Association (the "Association"), under the recommendations of the Canadian Hockey Association and the Ontario Women's Hockey Association, have adopted a screening process for volunteer applicants.  This screening process precedes final confirmation for any coaching appointment.  The process involves the investigation of personal information and requires your consent.  Any information obtained will be maintained in strict confidence and, if necessary, you will have the opportunity to discuss it with the Association.

As a condition of applying, all candidates agree to adhere to:
(a) the LDGHA screening process
(b) LDGHA Player development program
(c) LDGHA Code of Conduct
(d) LDGHA rules and regulations

I am interested in being considered for a sensitive position of trust and well being with the participants of the Association.  I hereby agree to abide by the constitution, by-laws, policies and procedures of the London Devilettes Girls Hockey Association.  I give permission to the LDGHA to use and release the information supplied above, to the extent necessary, to confirm its accuracy and to obtain information relating to my application to coach.  I further give permission to the City of London, London Police Services, Ontario Provincial Police and/or Royal Canadian Mounted Police to give to the London Devilettes Girls Hockey Association Board of Directors any and all information relating to any dealings I may have had with any police agency including any criminal record information I may have.
