House League Coaching and Volunteer Application 2025-2026 Season
Before submitting your application, carefully review the following expectations and requirements: All required fields must be fully completed for the application to be considered. The “Coaching Philosophy” section is mandatory. Applications missing this section will not be reviewed. The goal of our coach selection process is to identify the most suitable individuals to lead and support our athletes both on and off the ice. Please note that coaching positions are subject to ongoing evaluation. Coaches may be removed from their role at any time if they are not meeting the expectations set by the Board of Directors. Successful applicants will be expected to attend coach meetings and/or in-house clinics as required. Deadline: Applications for all teams will close on August 15, 2025 at 11:59 PM.

I give permission to these persons to provide information about myself to the London Devilettes Girls Hockey Association.
The London Devilettes Girls Hockey Association (the "Association"), under the recommendations of the Canadian Hockey Association and the Ontario Women's Hockey Association, have adopted a screening process for volunteer applicants.  This screening process precedes final confirmation for any coaching appointment.  The process involves the investigation of personal information and requires your consent.  Any information obtained will be maintained in strict confidence and, if necessary, you will have the opportunity to discuss it with the Association.

I am interested in being considered for a sensitive position of trust and well being with the participants of the Association.  I hereby agree to abide by the constitution, by-laws, policies and procedures of the London Devilettes Girls Hockey Association.  I give permission to the LDGHA to use and release the information supplied above, to the extent necessary, to confirm its accuracy and to obtain information relating to my application to coach.  I further give permission to the City of London, Ontario Provincial Police and/or Royal Canadian Mounted Police to give to the London Devilettes Girls Hockey Association Board of Directors any and all information relating to any dealings I may have had with any police agency including any criminal record information I may have.
